Shared Decision Making

Making decisions about your care with your doctor or nurse (shared decision making).

When you visit your doctor’s surgery you will often find that there are decisions to be made about your health and the treatments that might be available to you. This includes when you are choosing between different types of treatment or different ways of managing any condition(s) you have.  When these decisions are made it is important that you are part of that process, so that you are able to come to the best decisions based on what is important to you

For my information, please see the Shared Decision Making page under further information located on the right hand side of the page.

Paracetamol On Prescription

Clinical Pharmacist

All of our patients who are on repeat medication are expected to have medication reviews at least once a year. These can now be done by our new practice pharmacist, Karen. If you have more than 7 medications on your repeat prescription, please book a double appointment.

Accountable GP

By the end of June 2016 all patients (including Children) will be assigned a named, accountable GP. We will be informing patients of their named GP during the year by various methods or you can contact the practice if you wish to find out who your named GP is.

If you have a preference for your named GP please let us know and we will make every effort to accommodate this request although you can see any doctor at the practice and not just you’re named GP.

Surname A - M will be assigned to Dr C Chapman

Surnames N - Z will be assigned to Dr R Keogh

We would like to take this time to inform you of all our regular Doctors we have at Earsdon Park Medical Practice.
Dr Rebecca Keogh, Dr Claire Chapman, Dr Rahul Saraf and Dr Sarah Redford.
