Prescription Line

Please note we have changed our prescription times. Please call between 10am-12pm or 2pm-4pm.

Patient Online Services

Are you aware you can order prescriptions, book appointments and view your summarised medical record online. To register, please visit the Patient Online Services section located right of the page.

Earsdon Park Oxford Centre

Please be aware that our practice in the oxford centre is a branch surgery and will be limited on appointments.

GP Appointments

If you need to discuss more than one problem with the GP, please book a double appointment.

Shared Decision Making

Prescriptions

Please note we cannot issue urgent prescriptions unless it is asthmatic or diabetic medication.

 

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients must have a new patient health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
